Senior Practitioner
Pooraka (Full Time - 1.0 FTE)

 

  • Fixed term, Full time 1.0 FTE | Location:  Pooraka

Provide therapeutic case management to at-risk youth, support independence, mental health, and housing, and lead Youth Case and Support Workers.

 

About the program

Hannah Place is a residential program offering flexible accommodation and outreach support to young mothers. Each young woman lives in a purpose-built self-contained unit, with wrap-around support provided onsite. We also offer post-care services (SPC) up to age 21, helping young people transition into long-term housing and independent adult life.

About the Role

As the Senior Practitioner, you’ll provide case management and leadership within the program, working closely with Youth Support Workers. You’ll support young women and their children by developing and implementing trauma-informed, strengths-based plans to guide their journey toward stability and growth.

A Snapshot of What You’ll Do

  • Develop and lead Support and Safety Plans, including for children, in collaboration with DCP and clients
  • Provide therapeutic case management focusing on parenting, mental health, housing, and wellbeing
  • Supervise and guide Youth Support Workers and support case direction
  • Support young people with Moving to Adult Life and Independence (MALI) plans under SPC contracts
  • Build community links, coordinate services, and advocate for best outcomes through multi-agency collaboration

What You’ll Bring

  • A degree in Social Work (or be working towards registration)

  • Experience working in youth homelessness, mental health, or related fields

  • A calm, confident leadership style and strong communication skills

  • Confidence managing risk and complexity in a fast-paced environment

  • A valid WWCC and First Aid Certificate (or willingness to obtain)

We’d Love It If You Also Have

  •  Expertise in areas of mental health, substance misuse, domestic violence, homelessness and parent/child attachment and trauma responses
  • Knowledge of the homelessness sector and intersecting services and agencies
  • Experience working with young parents and/or young people.
  • Experience in providing case management supervision
