About the Role Centacare is seeking a motivated and compassionate Community Connections Coordinator to join our Community Connections Program (CCP). This program supports people aged 18 to 64 who are not eligible for mainstream services such as NDIS or My Aged Care, and who may be experiencing social isolation or disconnection from support networks. In this role, you will provide short-term, strengths-based support through both individual and group engagement, helping participants build confidence, develop connections, and improve their overall wellbeing. You will work collaboratively with a statewide network of partners to ensure participants are linked to appropriate supports and opportunities. Key Responsibilities - Provide 1:1 support to participants over a 12–24 week period, helping them set and achieve individual goals
- Build and maintain strong relationships with community partners and stakeholders
- Plan and deliver group activities and social connection opportunities, including culturally responsive programs
- Complete assessments, case notes, and data entry using DHS and Centacare systems (R2D2 and Lumary)
- Support program promotion and community engagement, including attending events and networking activities
- Apply culturally safe, trauma-informed and risk-aware practices in all aspects of service delivery
What you need to bring Mest have: - Minimum Certificate 4 qualification in a relevant field and demonstrated equivalent experience
- Demonstrated ability to build rapport, stakeholder and community connections
- Proficient in computer use and ability to learn and use databases
- Strong written and verbal communication skills Current WWCC
- Valid Driver's Licence
Bonus if you have these.. But not necessary - Tertiary level qualification in related field
- Experience working with Aboriginal and Torres Strait Islander communities
- Experience working with CALD communities
- Experience working with unpaid carers
- Experience working within the community services sector
