Make a lasting difference in the lives of young mothers and their children as they navigate the challenges of homelessness. The Role Centacare’s Young Family Support Programs has an opportunity for a dedicated Senior Practitioner to join our team at Malvern Place. As a Senior Practitioner, you will play a vital role in providing therapeutic case management to young women who are pregnant and/or parenting, and homeless or at risk of homelessness. This role is integral to the Young Family Support Service Model, which focuses on supporting young mothers and their children to build capacity, strengthen family relationships, and establish meaningful connections within their communities. - Malvern Place: Supporting young women up to 25 years old and their children in a crisis and short-term residential setting. The key duties in this role will include: - Deliver high-quality case management tailored to complex needs, addressing areas such as mental health, parenting, housing, and independent living skills.
- Develop case plans for children, considering the impact of homelessness on their health, attachment, and development.
- Supervise and guide Family Support Workers and Children’s Workers, ensuring effective, trauma-responsive care.
- Provide clinical leadership and supervision, promoting best practices in attachment, trauma-informed care, and strengths-based approaches.
- Maintain accurate records, including H2H data, and participate in professional development and multi-agency collaborations.
- Act as a key support to the Senior Manager, providing site coordination and support with program issues as required.
This is a full‑time, fixed‑term role until 30 June 2027, with the possibility of extension subject to government funding About You To excel in this role, you will bring: - A Social Work degree and demonstrated expertise in supporting young people with complex needs.
- Strong clinical knowledge in areas such as childhood trauma, attachment theory, and adolescent development.
- Understanding of the homelessness sector and intersecting services.
- Excellent communication skills, including written case notes and reporting.
- Leadership skills to guide and supervise team members, fostering a positive team culture.
- A proactive, solutions-focused approach to client care and program delivery.
- Hold valid Senior First Aid with CPR Certificate
