Help young people aged 15–25 access safety, support and opportunities through responsive intake and case management. The Role Centacare is offering a permanent full-time position for an experienced Intake Worker - Case Manager within our Domestic Violence unit. Reporting to the Manager of Homeless Services, you’ll work Monday to Friday, with an on-call duty every four weeks to support, guide, and provide debrief Support. As the Intake Worker - Case Manager, you will be directly involved in providing outreach and support to individuals, couples and families between 15-25 years who are homeless or at risk of homelessness. Services are located in the Playford and Gawler Council areas. Services are delivered in a range of accommodation contexts such as properties owned by SA Housing Trust, Community Housing or private rental, with support also being delivered to young people at home and in a family context. The key duties in this role will include: - Conduct Intakes and Assessments to determine needs
- Conduct ongoing risk and safety assessments for at risk young people
- Conducts assertive outreach within the community
- Provide crisis support to presenting clients with high risk and complex needs
- Providing case management to support clients and ability to manage a case load.
- Advocate for clients and work with government, non-government, and community agencies to achieve case plan goals
- Work alongside Adelaide North West Homelessness Alliance
About You As an experienced Intake Worker - Case Manager, you bring a deep understanding of homelessness and case management expertise. Your compassion shines through as you listen empathetically and non-judgmentally to each individual's story. Known for your exceptional communication skills and ability to build strong relationships, you thrive in collaborative team environments. Driven and solution-focused, you're committed to empowering individuals to overcome homelessness and achieve lasting change Qualifications and Skills: Essentials - Tertiary qualifications in Human Services (Diploma or higher) or extensive experience in the field; a University Degree in Social Work is highly regarded.
- Minimum 2 years’ experience in a similar role
- Proven crisis intervention skills and experience
- Demonstrated case management expertise, including monitoring, reviewing, and assessing client progress against case plans
- Experience working with young people with complex issues
Desirable - Experience in the homelessness sector and in similar organisations
- Experience in managing tenancies such as moving in and out of property procedures including leases and the application of the Residential Tenancies Act
- Demonstrated ability to coordinate service delivery on behalf of clients
