Provide person-centered, psychosocial rehabilitation support to people with enduring mental illness to enable them to live independently in the community.

About the Position
Centacare’s Individual Psychosocial Rehabilitation & Support Service (IPRSS) provides person-centered, psychosocial rehabilitation complex case management support, within a recovery framework, to people living with mental illness. Support Workers work with consumers, and families/carers where appropriate, undertaking joint service planning with Mental Health Services to provide strengths-based, goal focused and individually tailored services.
Support Workers identify consumer needs, aspirations and goals in key domains:
Supporting & promoting self-management, health and wellbeing,
Developing living skills & capacity for community living,
Family and social relationships, and
To be considered for this position you will need to meet the following criteria:
Certificate IV in Mental Health (non-clinical) or equivalent
Experience working with people with complex needs and/or people living with a mental illness
Valid Department of Human Services (DHS) Working with Children Check (previously known as ‘Child-Related Employment Screening Clearance’)
Valid Australian Driver's Licence

Willing and able to work outside of standard work hours at times if required
Degree in relevant Human Services discipline
Experience using Athena Penelope electronic client management system
