About the Position
Outer North Youth Homelessness Service provides services to young people aged 15 and up to 25 years who are at risk of homelessness or who are homeless and are based in The Barossa, Gawler and Playford regions. ONYHS are partners with Padninthi Kumangka Wardli Alliance for the North West.
The Case Manager will work collaboratively with community based services within the Barossa Council region which enables them to work together with young people who present for support and assistance to find appropriate transitional accommodation.
The following key responsibilities will ensure you contribute to the program meeting its objectives.
Conduct Intakes and Assessments to determine needs
Conduct ongoing risk and safety assessments for at risk young people
Conducts assertive outreach within the community
Provide support to presenting clients with high risk and complex needs
Develop and review case plans to achieve mutually determined goals
Daily entry of accurate and relevant case notes on H2h

To be considered for this position you will need to meet the following criteria:
Degree in Social Work or a related discipline
Experience in case management, and demonstrated ability to monitor, review and assess client progress against the case plan
Clinical knowledge of trauma informed approach
Experience working with young people with complex issues
Understanding of homelessness and related issues
Proven ability to understand complex issues and develop creative solutions to problems working alongside the client
Valid Department of Human Services (DHS) Working with Children Check (previously known as ‘Child-Related Employment Screening Clearance’)

Valid Driver’s Licence
Ability to participate in an on-call roster
Ability and availability to do some work outside of standard hours when required
Experience using H2H data collection systems
Experience in the homelessness sector and in similar organisations
Experience in managing tenancies such as moving in and out of property procedures including leases and the application of the Residential Tenancies Act
Demonstrated ability to coordinate service delivery on behalf of clients
