Case Manager (Outer North Youth Homelessness Services)
Elizabeth Park (Full-time)

Assertive outreach and support to young people aged 15-25 with high and complex needs who are homeless or at risk of homelessness.

 

The Role

The Case Manager will work collaboratively with community based services within the Playford and Gawler regions which enables them to work together with young people who present for support and assistance to find appropriate transitional accommodation.

 

This is a fixed term opportunity until 30 June 2023 (a subsequent fixed-term period may be offered subject to funding).

What you need to bring

  • Degree in Social Work or a related discipline
  • Experience in case management, and demonstrated ability to monitor, review and assess client progress against the case plan
  • Clinical knowledge of trauma informed approach
  • Experience working with young people with complex issues
  • Understanding of homelessness and related issues
  • Proven ability to understand complex issues and develop creative solutions to problems working alongside the client
  • Valid Department of Human Services (DHS) Working with Children Check. (previously known as ‘Child-Related Employment Screening Clearance’)
  • Due to the nature of our services, Centacare is required to comply with COVID19 government directions. You will be required to provide Centacare with your COVID19 vaccination certificate. If you cannot comply, you are encouraged to have a confidential conversation with the hiring manager about your individual circumstances to find out how this may apply to you.
  • Ability to undertake some work out of standard work hours when required
  • Ability to participate in an on-call roster
