The Role

Mental Health Workers work with people, and their families/carers where appropriate providing person-centred psychosocial support including goal setting, exit planning, and application and transition to NDIS services.

Your LINC provides the following support packages:

Intensive Crisis Support (ICS) (6 weeks) – provision of flexible high intensity case management support over a period of six weeks. The key outcomes are reduced levels of distress/increased wellbeing via addressing immediate stressors and crises, and supported transition and exit with step-down provision and warm referrals to services as appropriate.

Non-Acute Psychosocial Support (NAPS) (4 – 12 months) – provision of moderate intensity case management to support people to build independence, connect with natural and community supports, and improve people’s quality of life over a period of 4 months. NAPS clients who are likely to be eligible for the NDIS will be supported to test their eligibility, apply for access, and supported through planning and plan implementation to successfully transition out to NDIS services. An extension may be applied for up to a total of 12 months for NDIS applications and exceptional circumstances. The key outcomes are wellbeing via participation in recovery oriented and strengths-based social/recreational/vocational activities, and supported transition and exit to NDIS and services as appropriate.
